Collaborative Family Health Record (cFHR™) app for iPhone and iPad

The CareEvolution Collaborative Family Health Record (cFHR™) allows you to easily view and manage your familys medical information if you are a patient of a participating healthcare provider. Once you log in, your healthcare provider can send you helpful alerts about new results, upcoming appointments, or other important information relating to you or your family’s health. The cFHR fully supports the Apple Watch, allowing you to receive medical alerts and view your family’s medical records directly on your wrist.

The cFHR also features integration with Apple Health, allowing you to upload some of your Health data directly to your healthcare provider. When you enable Apple Health data sharing in the cFHR app, data stored in Apple Health will be shared with your healthcare provider automatically upon logging into the app.
